In accordance with G.S. 58-626, prior to the rending of any services by SERENITY & ASSOCIATES, LLC or their authorized representatives the hereby freely and voluntarily consents to a payment to SERENITY & ASSOCIATES LLC or a fee for processing all applications and issuing of policies. 

The amount of said fee shall be the following: 

-Personal auto application fee $20.00 

-Commercial applications fee $50.00 

-Bad check fee $25.00 

-Additional special forms for Commercial applications fee varies due to the type of request.
